:: General Exclusions

 

The General Exclusion terms of the Scheme are as follows:

• Treatment for Learning difficulties, Behavioural or Developmental problems.

• Chemotherapy and Radiotherapy of Cancer

• Complex major Surgery i.e. Brain or Heart Surgery

• Cosmetic/Plastic Surgery.

• Diseases arising from deafness or caused by congenital abnormalities, maturing or aging

• Illness form drugs/substance abuse and addictive conditions and disorders.

• Gynaecological investigation for infertility including in-vitro fertilization procedures

• Treatment for diseases not covered by the scheme.

• Treatment by a provider not on Premier Medicaid list.

• Treatment related to Kidney disease - Haemodialysis, renal transplantation

• Pregnancy and normal delivery within the first twelve months of a member's coverage

• Spectacles, Lenses, Visual and Hearing Aids.

• Treatment outside Nigeria without previous arrangement with PM

• Diseases resulting from Riot, Revolution, War, Nuclear and Chemical Contamination

• Embalming and related services.

• Treatment for HIV/AIDS

• Intensive Care Treatment for Renal Failure, Heart Failure and Myocardial Infarction.

• Artificial limbs and sophisticated Therapeutic Appliances
  CT Scan, MRI, Angiography.

NOTE:
Any of the above can be handled under 'PM Special' Plan.
